| Free Screening: |
We will start by measuring your prescription using computerized vision tests. A preliminary determination will be made as to whether you are a candidate for the procedure.
We will show you an informative computer presentation. You will then be offered an opportunity to schedule your procedure. |

| Scheduling: |
In order to schedule your procedure, you will be required to pay in full. We accept checks and major credit cards. We also offer an excellent financing program. At the time of payment, we
will assign you our next available procedure date and time. |

| Pre-Op Exam: |
Once you have paid in full and if you have had your contacts off for the prescribed time, we will be able to perform your Pre-op exam. This is a very extensive eye exam that gives us all the
information we need to perform your procedure. The Pre-op will take approximately 60-75 minutes. This will be a dilated eye exam. |
